According to the script treatment for the canceled Fallout movie, the actual instigator of the Great War was not the US, or China, or North Korea, or India - but Vault-Tec. The executive of the Vaults launched the first bomb to get things started and fulfill his own prophesy of nuclear annihilation. Obviously, this source is non-canon since the film never ended up happening, but it's still a cool idea.
