I'm thinking something, like, a game where you play as Peter Capaldi/whoever is the official Doctor at the time for the tutorial level, but then at the end of the level something happens that causes you to regenerate, which leads to this sort of Sims-style character creator where you make your own Doctor and figure out his traits, and then they drop you into this self-contained open world (say, London) which is small scale, but allows you to travel to various different periods, each with its own cast of characters and quests and stuff like that.

Edit: perhaps the way you handle your first post-regeneration quest would decide the sort of character you are and the activities available to you after that

As time goes by, patches, DLC, expansions, whatever would add stuff like new playable world spaces to the game so that the publishers can create sequels, add other additional content, or update the game with locations from the show's most recent season while still allowing the player to keep their character.
