@critialcloudkicker there are different kind of triggers in this game. Fathom Mage's ability may possibly only trigger if it sees a bigger creature etb. If the creature's already in and then got bigger upon entering via grafting, you *may* have missed the window. (You'll need to ask an active judge, I am totally inactive.) This scenario you've created is different from cards which create effects like "your creatures enter the battlefield with two +1/+1 counters," which would actually trigger Fathom Mage if the etb creature wouldn't normally do so by itself.
