@heartstring The only way to get astronaut stauts without actually trying. Believe me, test pilots are some of the most respected guys in the Air Force, because we all know that at some point, they might die, and it won't be their choice. Well, after breaking my ankle in BT, I had to have surgery on it (I basically have a metal ankle now) and I was still able to complete BT and continue on. So, I doubt they would have a problem with eye surgery... But i'll ask somebody about it, just to be sure.
